Rozporządzenie Ministra Zdrowia z dnia 11 października 2001 r. w sprawie zakresu niezbędnych danych gromadzonych przez świadczeniodawców, szczegółowego sposobu rejestrowania tych danych oraz ich udostępniania kasom chorych, Prezesowi Urzędu Nadzoru Ubezpieczeń Zdrowotnych, ministrowi właściwemu do spraw zdrowia i wojewodom, w tym także rodzajów wykorzystywanych nośników informacji oraz wzorów dokumentów.
