Rozporządzenie Ministra Zdrowia z dnia 30 kwietnia 2002 r. w sprawie opłaty za wydanie zezwolenia oraz dokumentów stwierdzających wpis do rejestru indywidualnych praktyk lekarskich, indywidualnych specjalistycznych praktyk lekarskich lub grupowych praktyk lekarskich.
