Rozporządzenie Ministra Rodziny i Polityki Społecznej z dnia 26 stycznia 2023 r. w sprawie wzoru wniosku o ustalenie prawa do jednorazowego świadczenia z tytułu urodzenia się dziecka, u którego zdiagnozowano ciężkie i nieodwracalne upośledzenie albo nieuleczalną chorobę zagrażającą życiu, które powstały w prenatalnym okresie rozwoju dziecka lub w czasie porodu
