Resolution of the National Electoral Commission of September 16, 2002, on establishing the forms of protocols and summaries prepared by territorial electoral commissions and certificates of election as a councillor and election as a mayor, city mayor and city president, used in elections to municipal councils, district councils and regional assemblies, the Council of the Capital City of Warsaw and district councils of the Capital City of Warsaw, and in elections of mayors, city mayors and city presidents.
