200–300 zł — Pay on the spot at the doctor's office. There has been no fixed official tariff since 09.2019 — the price is market-based, compare a few practices.
What you get
A medical certificate of no contraindications — issued on the spot. Note: the examination is needed only if your medical checks have expired or you are changing/extending categories; if you are only changing your surname or address, skip this step.

Apply to replace / renew your Polish licence

Collect your ready licence

What you get
Your Polish driving licence. When collecting it you hand in the foreign document — the office sends it to the issuing country (exception: privileged categories under § 14(12) — the document stays in the case file).

What can go wrong

An increase of the licence issuance fee to PLN 115.50 is in the pipeline. For now it is only a draft — as of 12 July 2026 it has not entered into force and the PLN 100 rate applies.

When applying, check the current fee amount on the office website or gov.pl — paying the wrong amount will delay your case.
