Where to go

Recent colour photo 35×45 mm — Any photo studio (ID-style: full face, light background); not older than 6 months.
Proof of payment of the licence fee (PLN 100) — Bank transfer to the office account (details on its website), on-site payment, or online via info-kierowca.pl.
Identity document (passport or residence card) — A valid document; shown when applying and collecting.
Medical certificate (orzeczenie lekarskie) — A doctor authorised for driver examinations (list at the voivodeship office); market price, usually PLN 200–300.

What you get

Your Polish driving licence. When collecting it you hand in the foreign document — the office sends it to the issuing country (exception: privileged categories under § 14(12) — the document stays in the case file).

What can go wrong

The loss must be reported to the starosta within 30 days. The loss declaration is made under criminal liability for false statements — do not report as lost a document you still have.

Report the loss without delay and apply for the duplicate right away — the lost document is invalidated and will not return to circulation even if found.
