Regulation of the Minister of Labour and Social Policy of April 6, 2000 on defining the form of the application for establishing the right to benefits from the maintenance fund and the form of the declaration of family income of a person applying for benefits from the maintenance fund, as well as the forms of other declarations necessary to establish the right to benefits from the maintenance fund.
