Regulation of the Minister of Labour and Social Policy of April 16, 2002, on determining the form of application for establishing the right to benefits from the alimony fund and the form of declarations necessary for establishing the right to benefits from the alimony fund.
Rozporządzenie Ministra Pracy i Polityki Społecznej z dnia 16 kwietnia 2002 r. w sprawie określenia wzoru wniosku o ustalenie prawa do świadczeń z funduszu alimentacyjnego i wzoru oświadczeń niezbędnych do ustalenia prawa do świadczeń z funduszu alimentacyjnego.
